1. Type your classmates name alphabetically.
2. Include their student numbers.
3. Type "w/ permit" (individually) for Student Permit No.
4. Fill in the details at the bottom part of the grade sheet such as:
Code Number -this is your subject code e.g. HS110
Course Description: (example) Heograpiya at Kasaysayan ng Pilipinas
Number of Units: 3
Year and Section: (include your course as well)
Time, Room, Semester and School Year
For the Signature of Department Head:
History Subjects, type: Prof. Angel Duque
Humanities, Literature Subjects, type: Dr. Miriam A. Padolina.
Note: Double check the names (spelling etc) please.
KINDLY PRINT IN QUADRUPLICATE (4 COPIES) YOUR GRADE SHEETS AND SUBMIT THEM ALL TO ME AFTER HOLY WEEK.
